Assistance Forms

These forms can be filled out to receive assistance in the Enterprise Community.

Request for Assistance Form - This form is filled out by new organizations requesting assistance from one of the 12 program activities of the NTAC.

Outreach Weekend Form (pdf file) - Outreach Weekends are held 4-6 times each year. Volunteers from the University of Illinois are matched with community organizations to implement community development projects. This form is filled out by all organizations requesting Work Weekend volunteer assistance. [word file]

Data Collection

Neighborhood Condition Survey (NCS) -The NCS collects parcel-level data on building condition, material, occupancy, evidence of fire damage, recent improvements, etc. The survey also collects data on the condition of the property, the number of trees, the presence of sanitation violations, conditions of the right-of-ways, etc. The data is collected using parcel maps, scantron forms and student volunteers. The scantron forms are quickly scanned and the coded data can be use in the creation of maps and for further analysis. ESLARP has used the NCS for the last few years as one way to access the physical condition of East St. Louis neighborhoods.

Resident Survey -Specific objectives include: gathering resident perceptions of current neighborhood conditions and services; gathering resident input on the desited goals and directions of their neighborhood plan so that planning efforts can be based on these desires; gathering resident input on resources and barriers that might serve to enhance or hinder the revitalization efforts; and enhancing community awareness of the revitalization project and their neighborhood organization to elicit community participation in future planning efforts. Cognitive Mapping Exercise -Long term residents, small children, business people and community leaders have vast knowledge of the community where they live, work, play and serve. The variety of information collected from the residents was the outer boundary of the neighborhood, district boundaries, local landmarks, resources or assets, and problems sites/areas.

Community and Business Leader Survey - This survey has three objectives: 1) to identify current community resources available to the neighborhood, 2) to determine perceptions of the current and future needs of the neighborhood, 3) to gather input on possible objectives and implementation strategies for the Neighborhood Improvement Plan.
